In a tragic circumstance and 8 month old child was killed when a storage rack fell on top of him. The company is offering a repair kit to purchasers of the storage rack. While the manufacturer is taking the high road in this situation could more steps have been taken to have prevented this tragedy?

Bark wrapped candles catch on fire.

I’m really not sure how this one slipped through the cracks but wrapping a candle in birch bark doesn’t really seem safe. The danger is that the bark can catch on fire as the candle burns down. Seems reasonable as when building a fire in the woods birch bark is considered GOOD tinder. [...]

Nike Recalls Football Helmet Straps

Football helmet straps have broken a reported 18 times. When the strap breaks it causes the helmet to shift or fall off and basicaly removing the ability to protect the wearer. This has resulted in some injuries ranging from concussions to deep cuts to a broken nose.
